Here you can book you Pfizer vaccination against Covid19 if you are 45 years of age or older

If you already received Covid vaccination outside of our surgery, please let us know by filling this form. We will ammend your records accordingly.
Please click “submit” button only once.
You will receive e-mail with confirmation of appointment after submitting the form. You will be also redirected to page with description of common side effects.
Please send only one form and check your e-mail (including SPAM folder) before assuming the booking did not go through.
Please note that below booking is based on HSE declaration to deliver sufficient amount of vaccines. Shall HSE fail to deliver, we might be forced to cancel appointments.
If you can not fill in the form because some of required statements are not true in you case (for example if you had anaphylaxis in past), please call the surgery.
If you changed your last name, please use the name under which we know you in surgery, so we could identify you in our IT system.
If there is no time option to be chosen and there is a note “no slots available” that means that no more appointments are available on given day.
Please choose only one option. Otherwise it creates significant workload to delete double bookings.
All booking made by people who are not our patients or are younger than currently vaccinated cohort will be deleted.
